If you would like care from a midwife at Cardinal Community Midwives, please apply (or register) on the Alberta Association of Midwives Application Site.


We use this site to find.accept all of our clients into care. Apply on this website, please make sure you add Cardinal Community Midwives as one of the practices who can see your application. You are welcome to specify one specific midwife from our practice, if you have a preference.

We also ask that you fill out this supplemental application form. This allows us to get to know you a little better and get a better understanding of your goals and desires for your pregnancy. This form is also used to get your chart started if you are taken in as a client. As midwives we prioritize home birth and out of hospital birth locations (as we are the only care providers that can offer that). We also prioritize north east, sherwood park, fort saskatchewan and surrounding locations. We also prioritize military members and their family and marginalized members of the community.

If you are selected for care, you will receive an email offering a meet & greet session. This is a brief 30 min session via zoom where we discuss midwifery care, what Cardinal Community Midwives offers, your history and goals and see if we are a good fit for you. This is a way to see if the midwife that is available will fit your needs, and see you fit the midwifery model of care. In this appointment, we will discuss next steps if we are a good fit.

Please be aware:

  1. We select our clients from the Alberta Association of Midwives Website but use the intake form to help guide us and give more information about you, why you want midwifery care and if you would be a good fit for our practice. This supplemental form helps you tell us more about you and why you want our care. (use it to your advantage!)

  2. Filling out this intake form does NOT mean you have been accepted into care. We can not take everyone and will send you an email for meet & greet or welcoming you to care. You are not in our care until you receive the “Welcome to care” email and book your first in-person appointment with us.

  3. Midwifery care is for low-risk healthy pregnancies

  4. Keep in mind what type of client and the client location we prioritize.

  5. If you are accepted to care we will send you a very detailed welcome to care email with all kinds of forms to fill out and appointment options for your first IN person appointment.
