If you're interested in receiving care from Cardinal Community Midwives, please follow these steps to apply:

1. Apply Online with Alberta Association of Midwives.

Begin by registering on the Alberta Association of Midwives Application Site, which we use to manage all client applications. Make sure to select Cardinal Community Midwives as one of the practices that can view your application. You may also indicate if you have a preference for a specific midwife from our team, helping us match you with the right care provider. We will only contact you if we have availability and can offer you a Meet & Greet session.

2. Complete the Practice Application Form

To apply directly to Cardinal Community Midwives, please fill out our practice application form. This helps us understand your pregnancy goals, preferences, and overall care needs. It also allows us to begin preparing your chart if you are accepted. We prioritize home births and out-of-hospital births and give special consideration to military members and marginalized community members. Again, we will only contact you if we have availability and can offer you a Meet & Greet session.

3. Next Steps

If selected for care, you will receive an email inviting you to an online meeting. This session will cover the basics of midwifery care, what Cardinal Community Midwives offers, and discuss your history and goals. It’s an opportunity to ensure we are a good fit and to outline the next steps if we proceed.

Important Notes

Selection Process: Completing the intake form provides valuable insights into your needs, but it does not guarantee acceptance. We cannot accommodate everyone, and you will only be officially accepted into care after receiving a "Welcome to Care" email and booking your first in-person appointment.

Midwifery Care Criteria: Our services are tailored for low-risk, healthy pregnancies. Unfortunately, if you have specific medical conditions, we may be unable to provide care. While we can manage many situations, our scope is specific regarding what we cannot accommodate.

Welcome Email: If accepted, you will receive a detailed "Welcome to Care" email with forms to complete and options for scheduling your first in-person appointment.
